This Account Profile consists of four pages (screens) and takes less than ten minutes to complete. You may want to have on hand your board member's full address and the date your organization was formed and date incorporated (year alone is acceptable). All other information is basic identifying information such as names, addresses and phone numbers.

Municipal agencies/departments and university/college departments should complete a separate Organization Profile for each agency/department/college that applies.

If you are using a Fiscal Sponsor, you must submit a letter signed by an authorized official of your Fiscal Sponsor organization agreeing to receive any grant funds on your behalf for this project, distribute them to your organization and maintain appropriate records.

  This must be listed as the exact legal corporate name the IRS recognizes and which you list on your W-9 form.
  Check this box if your organization is using a Fiscal Sponsor Definition.
  List the name of your Fiscal Sponsor organization:
  If you will be using a Fiscal Sponsor (G link) because you are an unincorporated organization; or you are a school but your PTO/PTA/APT will be managing the grant -- complete this profile for your organization (not your Fiscal Sponsor). You will fill in the EIN Number of your Fiscal Sponsor, and your Fiscal Sponsor must be listed as your Authorizing Official. All other information should refer to your organization. Be sure that you and your Fiscal Sponsor are familiar with the obligations for you both (click here for more information).

A D-U-N-S number is a unique nine-digit identifier for single business entities, which also links corporate family structures together. D-U-N-S numbers are free, do not open you up to solicitation or risk, and in today's global economy, are the standard for keeping track of the world's businesses. Although optional, we highly recommend your organization acquire, and provide us with, a Dun & Bradstreet D-U-N-S number to help measure the scope and impact of Arizona's creative industry.

Do not use symbols or punctuation. This will be the single username and password for all of your organization's applications and users, so it should be shared with anyone in your organization who needs to access EGOR. Please coordinate internally who will complete/update this Profile so that two users don't inadvertantly overwrite information.
